3. What have you been doing athletically since entering high school in Fall 2003?
4. Why did you delay your enrollment until August 2008 after graduating high school in April 2007?
5. Please list all track/cross country teams that you have competed for since entering school.
6. For all teams and events you listed above, please answer the following questions:
a. The name and location of the team(s) or event(s).
b. The time period during which you competed for the teams or in the event(s) (mm/dd/yyyy – mm/dd/yyyy).
c. The league(s) in which the team(s) competes.
d. The level or division of track/cross country each league or event is considered in your country of residence (i.e. highest league; 2nd highest league; etc.)
e. Name and e-mail address for a coach or other administrator with each team.

o. Does the team provide any of its athlete’s more than actual and necessary expenses? Actual and necessary expenses include: a) meals; b) lodging; c) transportation; d) apparel, equipment, and supplies; e) medical expenses or medical insurance; and f) stipend. If so, how many athletes receive this compensation?
p. What did you receive for free when competing with the team(s) or in the event(s)?
q. Did you pay a fee to participate with the team(s) or in the event(s)? If yes, how much was the fee, how often did you pay it and what was it used for?
